Good find on the MA Acknowledgment form

My first look at the MA notary law I couldn't find that so Googled and found the form I quoted from.

Starting here, under Section 15, is what you have and appears to be correct. I would have no problem signing it.

Double checked "Known to me" and "personally Know to me" and you appear to be correct there also.
