(1)The Colorado Department of Revenue has received your Withholding refund request. Your request for a refund of $632.00 for the period ending 12/31/2021 has been denied for the following reason(s): (2)• Missing an explanation of how payments totaling $632.00 were calculated if the withholding was $0.00.(2a) (3)• Our records show that the total 2021 wage withholding payments made on this account were $0.00 rather than the $595.00 reported on your 2021 DR 1093 - Annual Transmittal of W-2 Forms. Therefore, no overpayment credit is available and a balance of $595.00 plus penalty and interest for the tax underpayment is owed. (4) If you do not agree with the adjustment(s) to the requested refund, you must protest this letter. Protest and/or hearing request must be filed within 30 days from the date of this letter in writing. In short, this letter says:
