So many things wrong with this and I've seen state documents that are similarly formatted. I would have attached, but the document doesn't specify if an acknowledgment or a jurat is required. The state should know what notary verbiage is needed. Also, why wasn't this very important document reviewed by the signer's attorney first to confirm what was needed.
