If its the parents asking this for their child, I do ask
Posted by  Cari of IL on 8/23/16 2:36pm Msg #558933
that both parents come to our appointment or at least one parent, along w/ the child's birth certificate and the parents identification documents. If the child is traveling with a grandparent, aunt, uncle, they must come as well, and show me their identification. They all sign the Travel Document. I've had no objections to this.

We don't have a responsibility to verify anything, only to make ensure the person signing the travel document is/are the persons listed on the form after diligently checking their identification documents.

I keep copies of the travel document for my records and delete off of my cloud server after a year.
