| I do a lot of parental consent forms here in Minnesota and it is not your responsibility to verify the parent/guardian relationship. They are going to give you the form that is already completely filled out and each parent is going to sign in front of you and you are going to id them and fill out the acknowledgement portion on the form, sign it, stamp it, collect their money and wish them safe travels. The minor child is probably traveling with a birth certificate or paperwork that states who their parents/guardians are. |
