... whether a jurat or acknowledgement (affirmation) is needed. That would be Unauthorized Practice of Law (UPL). The Notary just fills out the notarial block provided. Period.

If the wording says "Sworn to and subscribed ...", then you've got a jurat, which, by definition, requires an oath. Don't over-think things. There's no need for "a jurat stamp"; the wording is already present.
