What is preprinted on the document?

If there is NO certificate there, then you do not need to notarize the signatures on the document.

If there IS a certificate and it begins with "Sworn to" (or affirmed), then you need to complete the JURAT. If the preprinted jurat is not state compliant, then you need to modify it or attach a loose jurat to the document.

If there IS a certificate and it states "acknowledged", then you need to complete the ACKNOWLEDGMENT. If the preprinted acknowledgment is not state compliant, then you need to modify it or attach a loose acknowledgment to the document.
