This is just for California notaries, although other states may have similar requirements of some kind.
I just ran across another situation where I've been asked to notarize an Advance Healthcare Directive (AHD) for someone presently in a Skilled Nursing Facility (SNF). I remembered reading about this here so looked it up, an found this thread: Msg #470211. It says that in these circumstances, an ombudsman is required to be one of the witnesses.
This is NOT included in our Notary Handbook, but is part of the Probate Code, so it's something all California notaries need to be aware of:
