practices - here's the 4 scenarios I've encountered: (1) my own - when I obtained my new license here last year, told DMV I wanted my name as is - first-middle init-last (married name) - okay - no problem done; (2) DMV told wife signer she HAD to use her maiden name as her middle name; (3) friend transfers license in Stuart - says he MUST spell out middle name completely (exactly opposite what I was told for my license where I was allowed to use middle initial) and (4) the other day - woman went for license and they said "we'll put your maiden name on here - she hadn't used that name in over 30 years.
