I can see where there may be differing opinions about this form. If I Google ds3503 and click on the PDF form, I come up with this: http://www.bing.com/ck/a?!&&p=4cfaf4f303beb7adJmltdHM9MTY4MDgyNTYwMCZpZ3VpZD0zNzk1ODIzZC1jOTE4LTYyMDAtMzRhMi05MDIzYzg3NzYzYTkmaW5zaWQ9NTIyMQ&ptn=3&hsh=3&fclid=3795823d-c918-6200-34a2-9023c87763a9&psq=ds3053&u=a1aHR0cHM6Ly9lZm9ybXMuc3RhdGUuZ292L0Zvcm1zL2RzMzA1My5wZGY&ntb=1
Please note the 3rd bulletpoint under IMPORTANT. That's what my decision is based on to NOT notarize using a Subscribing Witness. That bulletpoint, imo, makes it clear that the principle must appear before the notary.
On the other hand, just clicking on the following link: http://www.bing.com/ck/a?!&&p=4cfaf4f303beb7adJmltdHM9MTY4MDgyNTYwMCZpZ3VpZD0zNzk1ODIzZC1jOTE4LTYyMDAtMzRhMi05MDIzYzg3NzYzYTkmaW5zaWQ9NTIyMQ&ptn=3&hsh=3&fclid=3795823d-c918-6200-34a2-9023c87763a9&psq=ds3053&u=a1aHR0cHM6Ly9lZm9ybXMuc3RhdGUuZ292L0Zvcm1zL2RzMzA1My5wZGY&ntb=1 and then clicking on the link for the PDF form within that website -- it does not give the detailed instructions as the first link above. This second form also does not require notarization.
Again, jmo.
