Now that I've done more than scan over the form and actually READ it, I have to completely agree. (That's what I get for trying to respond late into the wee hours... Sorry for the potential misdirection!)
If the person is required to sign in front of the notary - also excluding the possibility of just "acknowledging" a previously applied signature - that pretty clearly excludes use of Subscribing Witnesses. And if that isn't clear enough, it's even further emphasized on the second page above the signature line, saying in bold, all caps: "STOP! YOU MUST SIGN AND DATE BELOW IN FRONT OF A NOTARY." Doesn't get more clear than that...
